        On the Cover: A 65-year-old man with chronic right shoulder pain refractory to conservative treatment presented to us. He described an episode of “trivial” trauma in the past. Left: View from superior medial portal shows a reverse HAGL (humeral avulsion of glenohumeral ligament). Note the capsule retracted medially allowing unobstructed view of the infraspinatus tendon. Right: Same view after arthroscopic repair of the HAGL. The repair was accomplished using a percutaneously placed posterior anchor while viewing from the anterior superior portal.
